He is uncomfortable about referencing his bodily functions because he does not know how to politly say what he needs to do.

A shooting license is a certificate that says that a person is periodically not responsible for their actions. Nurses were praying too loud and the men were not able to sleep, so Paul threw a bottle in the hall where the nurses were praying to shut them up. After the bottle was thrown, an inspector walked in the room and asked who threw the bottle and someone with a shooting license took blame for throwing the bottle so everyone could go to sleep. The men do not get in trouble when they do not act with proper decorum because they can just blame it on the guy with the shooting
