Family Assessment
Family is a set of interacting individuals related by blood, marriage, cohabitation or adoption who independently performs relevant functions by fulfilling expected roles (Edelman & Mandle, 2010, p. 173). The student was instructed to perform a family assessment. She chooses the Isaacs family to do the assessment on. Annette is 47 years old. She is a nurse practitioner, she is 5ft 5inches and weighs 290lbs. Howard is 46 years old. He is an over the road tractor trailer driver. He is 6ft tall and weighs 220 lbs. Jermaine is 18 years old and he is a student. He is 6ft 2 inches tall and weighs 330lbs. Jordan is 10 years old. He is a student as well he I 5ft 6 inches tall and weighs 140 lbs.
The student started her assessment with the family’s health perception. Annette and Howard both have type 2 diabetes and Jermaine was diagnosed with type 2 diabetes within the past 30 days. Jordan was diagnosed with ADHD within the last month and was placed on medication. Annette is a nurse and knows she has to make lifestyle changes to get her family back to optimal health. A. Isaacs (personal communication, March 27, 2012) stated” I have been cooking healthier less fried foods and more baked foods. I also have been trying to get the boys to eat more vegetables which is not easy”. She also stated a desire to walk more. The student inquired about the families eating habits and was informed that Jermaine drinks a lot of diet soda more than 2 liters a day. Jordan drinks sugar free Hawaiian punch more than 4 cups per day. Annette drinks crystal light and Howard drinks water. The family is in agreement that their portion sizes are too large. and they rarely eat dinner together as a family. Howard is gone for weeks at a time working. The boys usually eat at the table together, and Annette usually eats her dinner after work on the sofa while she is watching T.V.
This family has a very sedentary lifestyle. Their only exercise is walking with the exception of Jordan. He loves to go outside and play. He also loves playing Xbox Kinect and the Wii. By trade Howard leads a very sedentary lifestyle, driving trucks for a living. Family fun time is also sedentary which consist of going to the movies, watching television and playing games. The family was encouraged to try and eat together at least two nights per week and to continue trying to incorporate vegetables in the meal plan. The family was also encouraged to grocery shop and cook together when possible. The family was also encouraged to increase their activity level for example riding bikes and going to the gym.
Since Jordan was placed on ADHD medication the student want to see if Jordan was having any side effects from the medication Vyvanse. A. Isaacs (personal communication, March 27, 2012) stated “Jordan was placed on medication because he could not focus in school and his grades were suffering. He has done well with the medication; he is able to sleep and has no stomach problems or diarrhea”. The family Bowel and bladder habits are within normal limits with the exception of Jermaine. His mother noticed he is frequently waking up at night to go to the bathroom which is affecting his rest. Jermaine sleeps 6 hour per night at most. Jordan sleeps about 9 hours per night, Annette 7 hours a night and still complains of fatigue. Howard is hard to assess he sleeps when he can. When he is home he sleeps at least 8 hours a night.
All members of this family wear corrective lenses. Annette is very consciences about her family’s annual eye exams. The family functions as a unit with mom and dad making all the major decisions. Annette communicated her word carries more weight because she is the one at home. The family overall appears to be happy with one another they are close and financially stable. A. Isaacs (personal communication, March 27, 2012) stated” at times I feel like a single mother because Howard is gone so much working.” She also feels very isolated in her own community. The Isaacs family lives in a diverse community but it is not a lot of African Americans in the community. Her neighbors of the past 4 years do not speak when they see her.
Since the family has one young adult and soon to be a teenager they are very open and honest when talking to their children about safe sex and sexual relationships J. Isaacs (personal conversation, March 27, 2012) stated ” My father and especially my mother preach to me all the time about safe sex and condoms”. Jermaine has a girlfriend and is still a virgin at this time. Jermaine is very concerned his friends will tease him if they find out he is still a virgin. The student counseled Jermaine to be proud of the decision he made to stay a virgin. The student also reassured him their conversation will remain private.
This family for the most part communicates by talking to one another; however Howard is the disciplinarian of the family. H. Isaacs (personal communication, March 27,2012) “In my culture (he is from Guyana) when a child gets out of line you have to deal with them right away. My father always said children are like trees you have to bend them while they are young into the shape you want them to grow because once the tree takes root it will be too late”. He feels as the husband and father he is in charge of the home as these beliefs are taught to them in church. Religion is very important to this family. They go to church every Sunday and when difficulties arise they lean on their faith and each other to see them through.
For this family there are a few family diagnoses that come to mind, imbalanced nutrition more than body requirements, risk for imbalance nutrition more than body requirements; Jordan due to mother and brother eating habits. Sleep pattern disturbance; Jermaine, fatigue Annette due to her weight and ineffective community coping.
Upon reviewing these diagnosis there is definitely areas for improvement,modifying their life styles by exercising, proper diet and keeping blood sugar at target levels. Jermaine will not urinate so much allowing him to get proper rest at night. Annette will not be as tired and as she loses the weight her body image and self-esteem will improve. As far as the community she is encouraged to go to the clubhouse or pool to interact with her neighbors.
Families are as different as night and day. What makes one family strong could stop another one dead in their tracks. If you’re lucky your family holds a bond that can never be broken. They are the one group of people whom you can trust with your life and they can trust you with theirs as well.

...Shaken baby syndrome is often seen in infant under the age of 1year. Shaken baby syndrome is caused by forcefully and violently shaking a baby. SBS is a leading cause of child abuse deaths in the United States. Babies (newborn to 4 months) are at greatest risk of injury from shaking. Inconsolable crying is a primary trigger for shaking a baby. Physical signs of abuse do not always occur with shaken baby syndrome. However, an X-ray or eye examination by a doctor may show signs of trauma. A doctor may detect rib fractures or notice bleeding behind a baby’s eyes.
