Hello Mr. Brown, I believe your visit to the clinic today is regarding your cholesterol results. I am Bernice and I will be your physician assistant today, I will be talking to you about your result and what the numbers of your results were. I will also be addressing with you any questions or concerns that you may have and I will try to explain everything to you so that you are fully informed on what this test consisted of. I am here to help you understand what cholesterol is and what changes we can make to improve your health.

I will begin by explaining to you what Cholesterol is and how it is produced. Cholesterol is a type of fat that can be found in the body and is a waxy type of material (Heart,2017). Mr. Brown, you may be wondering where and how cholesterol is produced. Well let me explain to you that cholesterol is produced by two things, one of those is your liver and also the food you eat. brown I will begin by reading to you your results if you have any doubts or questions feel more than glad to ask me. Now if you have no questions at the moment I will begin, your results were the following. Your triglycerides number came back as 145 mg/dL which is below 150 mg so that isn’t too bad but you will want to continue checking to see that it does not get higher. Secondly your HDL as known as good cholesterol was below normal range, your result was 33 mg/dL which is lower than what we would like it to be. Now Your LDL which is bad cholesterol came to be in the higher range and as we recall to earlier in our conversation I had mention that we strive for these number to be in the lower range rather than in the higher. Unfortunately, your result came back as 160 mg/dL which is what we would consider high. Last of all your Cholesterol level came back as 210 mg/dl, which is also considered high being that the normal level in which we encourage patients to be in would be 200
