Community Assessment Reflection
Community Health Partnerships
It is so important for different agencies in the community to partner with each other, this helps ensure that people get directed toward the community resource they are in need of. In my families community The Clay County Health Department does eligibility assessments and referrals for social services in addition to all the services they provide themselves (Clay County Health Department, 2011). Clay Behavioral Health Center partners with Clay County Health Department, Department of Children and Family Services, United Way, and Kids First Florida. They provide adult and child mental health counseling as well as outpatient alcohol and drug treatment (Clay Behavioral health Center, 2011). Quigley House is a 24 hour shelter for abused and sexually assaulted women and their children with a range of services up to and including transitional housing (Quigley House, 2012). The Council on Aging of Clay County partners with AARP, Community Hospice, Haven Hospice, and Eldersource, The Council provides transportation, adult daycare, home delivered meals, surplus foods, and respite care services (Council on Aging of Clay County, 2012). All of these agencies are important, integral aspects of a community.
Cultural Diversity and Vulnerable Populations
The health department is a great source for health care in the community they do not discriminate against anyone for any reason and their fees work on a sliding scale according to income. All of these agencies offer their programs to all cultures and their services are free or sliding scale based. Several of them set up booths at the local different cultural fairs to spread the word about their services. This community has Black Expo, The Spanish Heritage Festival, and The Clay County fair.
Epidemiological Influences
One of the big socially influenced problems in this community is adult and childhood obesity, which can lead to so many other health problems like Diabetes and Hypertension. This winter has been so mild that the Air Quality Index has been extremely high as it relates to pollen and it has led to an increase in asthma problems in the community. My family’s son has problems with allergies and his pediatrician told his parents this year has been one of the worst he has seen for allergy related problems in the winter.
Healthy People 2020 LHI
I chose TU-3 Reduce the initiation of tobacco use among children, adolescents, and young adults; because both of the parents in my family smoke, though not in their house because of their children. The goal is to reduce the use of tobacco products among children and adolescents ages 12 to 17. This includes cigarettes, cigars, and smokeless tobacco products. The Clay County Health Department offers smoking cessation programs, which could influence positively this LHI (Healthy People 2020, n.d.).
Nursing Diagnosis and Interventions
The three nursing diagnosis I chose are; 1. Readiness for enhanced family processes, which means the family’s performance is efficient, there is a good sense of well-being, and they are ready to move to the next level. The interventions are (a) Assess the family’s stress level and coping abilities during the initial nursing assessment. This helps assess the baseline stress level and coping responses of the family as a whole and individually. (b) Discuss with family members how they have handled previous crises. This discussion aids the nurse in her plan of care (Ackley & Ladwig, 2008).

2. Readiness for enhanced community coping, which means community activities and problem-solving skills are satisfactory but could be improved for management of stressors. The interventions are (a) Demonstrate optimum use of the power resources. This supports coping. (b) Assist community members with cognitive skills and habits of mind for problem solving. The cognitive skills and habits of mind of critical thinking support problem-solving abilities. 3. Deficient Diversional activity, which is decreased stimulation from or interest in recreational activities. The interventions are (a) Encourage a mix of mental and physical activities. (b) Discuss activities with clients that are interesting and feasible in the present environment (Ackley & Ladwig, 2008).
This assessment shows how some of the different agencies in the community work together for the common good of the members of the community. We may never need to use all of the resources available to us, but it is good to know about them should family or friends ever need them. My family is learning about available resources as well as volunteer service opportunities that are available for their family can get involved in so they can give back to the community.
