The purpose of this memo is to remind managers of the content of verbal and written warnings. It is important to document employees’ files thoroughly to avoid lability being found in lawsuits. The Corrective Action Form is found in our compliance management program Continuity. The following information includes the instructions and information to be included on this form.
Instructions Corrective Action Form:
• Employee - Please add the employee’s name who is receiving the warning
• Warning Delivered on - Provide the date the employee received the warning
• Employee’s Title – Provide the employee’s title who is receiving the warning
• Effective Date of Warning - Add the date the infraction occurred
• Department - Provide the department
