Adopting a family systems theory, which can be defined as understanding people not in isolation, but as part of their family can help shift the current systems theory, which fails to see the youth as part of a larger family entity and translates the youth in how they fit into the “system”. Currently, there is a model of intervention for youth with first episode psychosis called Early Psychosis Intervention (EPI) program in British Columbia that works to create a “care pathway” (Fraser EPI Program, 2017). In this program, youth and adolescents have an individualized plan created to provide “a practical "best practices" guide to care that fits within the system in which care is being offered, standardize care and reduce variation in practice …show more content…
Using best practices, a person-centred plan of care can be created to bridge the transition between the two service umbrellas. A central need would be adequate funding for this program. A second need recognized by the Mental Health Commission of Canada (MHCC) (2015) includes the creation of a “fully integrated system” (p.17) working towards meeting the needs of transitioning youth and young adults.
“Good transitions should be a coordinated, purposeful, planned and patient-centred process that ensures continuity of care, optimizes health, minimizes adverse events, and ensures that the young person attains his/her maximum potential. It starts with preparing a service user to leave a child-centred health care setting and ends when that person is received in, and properly engaged with, the adult provider” (Singh and Tuomainen, 2015, p.358). A shared management model as an acting continuum of care would address the very apparent lack of communication that currently plagues the transition from CYMHS and AMHS. Capelli et al., suggest a shared management model was implemented to address the service gap in transitional care. It is characterized by a flexible, collaborative approach whereby youth and adult providers from hospitals and communities work together with youth and families to develop individualized transitional care plans that promote coordination and continuity of care between CYMHS and AMHS. Adopting a family theory to the practice of transitioning youth would shift the practitioners thinking to incorporate family in the planning. Having natural supports on board with the planning will aid in better outcomes for the transitioning youth. “Family engagement is increasingly understood as the ideal model of service delivery” (Ontario Centre of Excellence for Child and Youth Mental, 2016, p. 3) In addition, either of these solutions require that there is adequate funding and staffing to enact the transitional programming that is cross-organizational and cross-ministry to create operational opportunities that will best meet the needs of the transitioning
