...Assessment of Non-acute Health and Social Services in Ponchatoula, Louisiana Laura K. Ogden Southeastern Louisiana University College of Nursing and Health Sciences School of Nursing NLAB 334 October 29, 2014 Assessment of Non-acute Health and Social Services in Ponchatoula, Louisiana The community assessed was Ponchatoula, Louisiana. The focus of the assessment was non-acute health and social services. The community-as-partner model was used as a guide in understanding how the different parts of Ponchatoula’s community make up its core, and to apply the steps of the nursing process (Anderson and McFarlane, 2011). The assessment was based on observations of the community, interview data from a nursing home director, and selected published data related to the healthcare and social services assessed. Assessment data was analyzed, and conclusions and inferences drawn from the analysis will be presented in the paper. Windshield Survey On October 28th at approximately 4:00 pm an observation of Ponchatoula was conducted while driving through the city’s limits. This survey was done to acquire an overall health assessment of Ponchatoula’s environment including buildings and people out and about. The city itself appeared dated as evidenced by the architecture, especially on E. Pine St. However, for being the age they are, the buildings appeared well kept with clean paint jobs. Sidewalks lined each side of E....

...The Community I chose is located in Marion County, Indianapolis, Indiana. The population of Indianapolis, Indiana in 2013 is listed as 843,393 (CityData, n.d.). Indianapolis is the host of several major events every year which includes the Indianapolis 500 mile car race; the Brickyard which hosts the Allstate 400 Nascar race; the Indiana Black Expo Circle City Classic football games bring some of the best black college teams to our city every October; the Indiana Black Expo Summertime Celebration at the convention center downtown and the NCAA Mens Final Four Basketball Tournament which is one of the most popular college events that the city hosts every year. These are just a few of the events that Indianapolis host each year. The community I am observing is primarily composed of blacks and is in the 46202 zip code. It is located approximately six blocks east of a well “manicured” neighborhood known as the Old Northside and, approximately, a ten minute drive to downtown Indianapolis. Several of the neighborhood residents state that in the past 1012 years, there has been a slow influx of whites moving into the Old Northside neighborhood due to the construction of new houses, condos and renovations of older homes which. At one time, blacks were moved out of those same houses because they were deemed unlivable and were condemned. Needless to say, a lot of those houses were never condemned but, were boarded up for 1520 years until they were bought by contractors who renovated ...

...Family Nursing Diagnosis and Assessment September 3, 2012 University of Phoenix Nursing 405 Ginger Witherington After spending time with my family and assessing their daily activities it is very apparent that they all need help coping and finding a solution to the growing problem within their home. By using the Friedman Family Assessment as a guideline to identify the structural framework of this particular family, nursing diagnosis and interventions can be drawn as a guide to help with the healing process of their family. According to Healthy People 2020, one particular goal is the “Reduce substance abuse to protect the health, safety, and quality of life for all, especially children”. (Healthy People, 2020). Alcohol and drug abuse is a pattern that is accompanied by failure to fulfill responsibilities such as work and family and can have harmful personal, physical and emotional consequences that lead to the downfall of family. The population that I believe most effected by substance abuse in the family is the children. When there is long term abuse, complications that arise are violence, domestic abuse and fatal accidents such as overdose or the results of drinking and driving. From the nursing standpoint there are several interventions and strategies that can be instrumental in helping a family in crisis. Giving necessary support, providing educational resources and counseling resources are just three small areas that are important for the nurse to provide...

...Course objective 1: Use beginning critical thinking and clinical judgement when providing safe, quality nursing care This objective was evident when working with the unit’s RN. I was able to watch him drain a patient’s chest tube. The man has two chest tubes to drain fluid that accumulates in his lungs. The nurse had me take his vitals and it was shocking for me to discover that his respiratory rate was 44 breaths per minute. This patient was certainly far more complicated than others in long term care. I was able to compare the patient’s current condition both before and after draining his chest tube. The nurse drained 1000 CC of fluid and the patient lost nearly 2.5 from this. Additionally his respiratory rate went down and his breath sounds improved. This...

...Ladder Application Exemplars – Level III Level III Proficient SUBMITS exemplar describing collaborative, functional relationships with members of the nursing team I truly strive to provide our patients with the best patient-centered care experience during their recovery. To complete this task most efficiently, I have recognized that being a team player and professional development is extremely important. I begin my mornings with rearranging the rooms in preparation for the surgical and medical admissions. I have found this to be most effective in reducing the stress of my coworkers during high volume admission periods. It also provides incoming patients with a smooth transition onto the floor. My co-workers and I, enter the rooms together ensuring that AM care is offered and/or completed and that the patients ‘overall needs have been met. Before I leave each room, I ensure that the phone, call bell and the patient belongings are within reach. I also make sure that all tubes, wires, and/ or drains are untangled and in the appropriate place. I document and complete hourly rounds in a timely manner. Unexpected findings, such as substandard vital signs/glucose levels, changes in patient conditions, questions and concerns from the patient/...

...I performed my health assessment at Bethany Nursing Home. The health assessment took place in an apartment inside of the nursing home. I wasn’t up on the floors where the residents need help with daily chores and tasks. The apartment was on the first floor where the people that reside in them are independent for the most part. The apartments are very nice and spacious and have the helpful tools, such as side rails to help them not fall. It is nice that they get to decorate their own apartments. While I was in my client’s apartment it was very easy to do my health assessment. I sat on her big coach with her and asked her questions. She even had a desk that lifted up, so I could write on. She said it was her husband’s desk. It was very easy...

...A written assignment that evaluates comprehensive approaches to nursing assessment for an older person with multiple health problems. The account should reflect on how the multidisciplinary team and other agencies including the family, may contribute to these assessment approaches. Particular attention should be paid to national policies in this area and evidence of best practice. The patient that will be discussed within this assignment is a 70 year old male admitted to hospital following a stroke. Pressure ulcer risk assessments and nutritional risk assessments will be discussed within this assignment looking at how they are used in the assessment of an older people with multiple health problems. A stroke occurs when there has been a disturbance or cut off of blood flow within the brain, this disturbance damages or destroys brain cells preventing these cells from doing their job. Damage to the brain causes problems with bodily functions and can also affect mental processes (Stroke Association, 2010). A stroke can cause sudden weakness or paralysis which often only affects one side of the body and is one of the most common symptoms of a stroke. Assessment 2 Post-op Case Study Assessment 2 Question 1. Identify a minimum of 5 nursing actions, in order of priority you would perform related to above information. Mrs Abu has had a considerable change in her vital signs (blood pressure lowered, her pulse is rapid, her respirations increased and temperature has dropped) form the baseline taken before surgery. These findings alone would be reported to the Registered Nurse and monitored. But because of the changes in vital sings, coupled with Mrs Abu reporting light-headedness and nausea, plus her significant blood loss form the surgical wound, you would be assessing for hypovolemic shock which can be life threatening. As the nurse you would be seeking assistance immediately, assessing her airway, breathing and circulation. Applying oxygen, applying pressure at the surgical site and continuing to monitor (airway, breathing, circulation) and vital signs until help arrives. Mrs Abu should be given nil by mouth as she may return to surgery (Gulanick, Myers, Klopp, Galanes, Gandishar & Puzas 2003, p.329). Question 2. ...Running head: Transcultural Nursing Assessment Transcultural Nursing Assessment Deborah Hill St. Francis University Abstract Giger and Davidhizar’s Transcultural Assessment Model is a valuable and functional assessment tool that evaluates the different cultural variables and how those variables effect health, illness and behaviors (Giger, 2013). This philosophy considers the uniqueness of each individual, understanding that the individual is unique, a product of their culture, religion, environment socioeconomic status and diversity. Giger and Dividhizar propose that, as health care providers, we need an acute awareness of the ethnicity and culture of each individual, having the knowledge and understanding to care for them as their culture, religion, values and belief system necessitates (Giger & Davidhizar, 2002). This model of nursing assessment focuses on six factors, or phenomena, that address important areas of cultural influence which impact our ability to provide diverse clients with the most effective and efficient care. The six phenomena are communication; space; social organization; time; environmental control; and biological variations. This assignment involves the use of the Transcultural Nursing Assessment model in the care of Ms. B, an 82 year old, African American female. Introduction America has been called the melting pot of the world. The term indicates that America is a county made up of immigrant cultures, religions and various ethnic...

Now the course assessments are formulated based on the course intended learning outcomes and course activities. These assessments could be of any type, including the formative, summative, direct or indirect methods of assessment, nevertheless they should be directly aligned with the intended learning outcomes and be the indicator of the student’s learnings. With the standard rubrics the course activity could become a form of assessment. It was observed that the alignment results in student’s and teacher’s satisfaction. The alignment gave students a deep learning of the topics under discussion rather than just superficial understanding (Biggs 1996, 2010; Ascough 2011: Wang...

...Culture Paper I was born and raised in the Philippines. I identify myself with the Filipino culture. I chose to learn about the Mexican culture, so I have interviewed my coworker. The interview was focused on the meaning of food in their culture and its impact on their health. When asked about the meaning of food, Estella and Betty both associated it with family gatherings, holidays, and celebrations wherein food brings people together. They celebrate Christmas, Quinceneras (when a girl is considered a young woman on her 15TH birthday), and weddings. Most of their meals include beans, rice, and either corn or flour tortillas. Tamales are commonly served during their celebrations because they have been their traditional food for many generations. Tamales are made up of corn meal, lard, chili, salt, and a choice of pork, cheese, raisins, or vegetables for the filling. They are wrapped in corn husks to keep them moist, and they are steamed. Some people from Mexico use lard in making flour tortillas and in making the masa (dough) of the tamales. Others also use lard for frying. Mole, a type of sauce poured over chicken and pork, is made up of melted chocolate, roasted nuts, and bananas. This type of dish is prepared and served only on special occasions because the preparation time is long. Refried beans is a common side dish. It involves a three-step process. First, the beans are boiled with salt. The second step is the mashing and frying of the previously boiled beans. The...

...Forensic Learning Disability services are specialist services for people who have a learning disability (LD) and/or have mental health problems who have been arrested, who are on remand or who have been to court and found guilty of a crime (Foundation for People with Learning Disabilities, FPLD, 2012). There are high, medium and low secure units all of which need to be high quality, patient centred and recovery orientated (Joint Commissioning Panel for Mental Health, JCPMH, 2013). There is a need for forensic health services as the JCPMH (2013) claim that patients in these services have complex needs and these needs typically cannot be met by commissioning generic health services. The time spent in forensic units depends on an individual’s recovery and progress towards rehabilitation. However, although the public protection remains the priority, there is a increasing consensus that forensic inpatient services may not always be an appropriate environment for those with severe mental illness and that detention can exacerbate mental ill health, increase the risk of self-harm and suicide and heighten vulnerability (The Institute Psychiatry, Psychology and Neuroscience, 2015). The Royal College of Psychiatry (2013) claim that people with a LD who come into contact with specialist learning disability mental health services often have a complex mix of LD, mental illnesses, personality disorders, substance misuse, other developmental and/or physical disorders including epilepsy...

...Reflective Essay The aim of this essay is to reflect on an incident, which took place in a hospital setting during the first month of my Foundation Degree Assistant Practitioner course. It will explore the importance of communication amongst the health care professionals and how a good nursing documentation is an integral part of nursing. It will also demonstrate how reflection enabled me to make sense of and learn from this experience, as well as identify any further learning developments needed to improve my practice and achieve the level of competency needed for when I qualify as an assistant practitioner. While discussing the knowledge underpinning practice, evidence based literature will be reviewed to support my discussion and for the purpose of reflection the essay will be written in the first person. Spouse, J, et al (2008). Jonhs, C (2009) defined reflection as learning through our every day experiences, towards realising one’s vision of desirable practice as a lived reality. He also added that it is a critical and a flexible process of self inquiry and transformation of being and becoming the practitioner you desire to be. However, Ghaye, T et al. (2000) stated that for the health care professionals to develop a more reflective posture, they must fully embrace both the principles and the practices of reflection. It is about becoming more aware of how we learn and how this affects what we think, feel and do. There are different models for reflection; some are more...
