Brief description of the company

4. Nature of work: Out enterprise has been formed mainly of coal, electricity, chemical industry, metallurgy, machinery manufacturing, and other multi-industry. We work in the mine which is a accident-prone area, so that our work has a certain degree of risk.
5. Roles: Employees should perform work under the direction of their employer. Employees are entitled to have superannuation contributions paid into a nominated superannuation fund by their employer. Policy statement
1. OHS policy statement Definition: Occupational health and safety (OHS) refers to the legislation, policies, procedures and activities that aim to protect the health, safety and welfare of all people at the workplace.

Aim: The aim of establishing OHS policy is to enable effective co-operation between the employer and employees to achieve a safe and healthy workplace. The objective of the policy shall be to prevent accidents and injury to health arising out of, linked with or occurring in the course of work by minimizing, as far as possible, the causes of hazards inherent in the working environment.

Requirements: Enable effective co-operation between an employer and its employees Establishment according to reality and be practical Arrangement relating to risk management Continuous consultation processes can be implemented smoothly

Process of establishment: 1. Hold an OHS consultation. (OHS consultation requires more than an exchange of information, it requires employers and employees to contribute to the pool of knowledge that can lead to quality decision making and improve health and safety at the workplace. Consultation means to appropriately invite and consider employees responses. A consultative approach will assist in defining the context and ensure different views are considered during a risk assessment process. Involving staff at all levels in the consultation process allows for ownership of risk identification and treatment of those risks. It also encourages endorsement for the process and the outcome.)

2. Make the OHS policy. (After consultation between employees and employers, the content of policy can be decided through all-sides considers. The policy should comply with national laws and the total policy of company.)

3. Take OHS management arrangements. (OHS arrangements must enable effective co-operation between an employer and its employees; promote and develop appropriate measures to ensure the health, safety and welfare of employees at work; provide adequate mechanisms for informing, reviewing, varying the arrangements and dealing with disputes that may arise in the course of consultation.)

4. Choose OHS employee representatives. (Employees can choose to be represented during consultations regarding HSMAs by either another employee, or by an employee representative. OHS employee representatives should do tasks and follow OHS policy as conscientiously as possible to set up a typical example for all the other employees. The representatives can help induct and train employees have some problems about health and safety. Also, they can report the problems to the employers.)

2. Roles and responsibilities of employees

Responsibilities: 1. Employees must take all reasonably practicable steps to ensure that their action, or omission ,do not create a risk, or increases an existing risk, to their health or safety or to the health and safety of other people at or near the workplace. 2. Employees are required to co-operate with their employer or any other person holding a duty under the act to the extent necessary to enable them to fulfill that duty. 3. Employees are required to use equipment, in accordance with any instructions given by their employer, consistent with its safe and proper use, and in the manner necessary to protect their health and safety or the health and safety of others at or near the workplace.

Roles: 1. Stick to jobs, conscientiously study and strictly observe safety rules and regulations, comply with labor discipline, no violations of safety rules, directly be responsible for the safety of the post. 2. Wear proper labor protection products, rationally use personal protective equipment. 3. Strictly carry out safety technology regulations, command signal should be correct and clear in order to prevent accidents 4. Inspect equipments before work to ensure the safety factor. 5. Have the right to refuse to violations of safety roles. 6. Once accident happens, taking immediate emergency measures to rescue

3. Training, instruction and induction processes relating to OHS

1. Introduce the new workers to their colleagues 2. Provide an organization plan (preferably with staff names and phone numbers on it), explain how the work unit operates, and explain how your work until fits into the overall organization. 3. Give them instructions (preferably written) and provide training to enable them to operate any plant or equipment they may be required to use 4. Advise where they can obtain items required for work (including any items of personal protective equipment (PPE) that may be necessary, such as steel-capped boots, earmuffs of safety glasses) 5. Show them the location of emergency exits, toilets, light switches, amenities rooms, parking, other facilities the may need to utilize 6. Provide the inductee with a copy of the organization’s health and safety management arrangements and associated OHS policies. 7.Discuss emergency procedures, including the location of fire escapes, emergency marshalling/meeting areas, fire extinguishers, what to do in threatening situations 8. Give them a written telephone list for staff members on the emergency control organization, which should include fire wardens, first aid officers, and similar roles 9. Provide information and instruction where necessary on anything in the workplace, including items of plant or hazardous substances, which may pose a threat to the employee’s health or safety 10. Introduce the new worker to staff with defined health and safety responsibilities, such as the health and safety representative and where possible, the OHS advisor and manager.
4. Safety audits A safety audit is a regular, systematic inspection of the workplace to evaluate the implementation and effectiveness of the organization’s health and safety system. We will keep up to date with government requirements regarding safety, safety equipment, reporting procedures. Also, we will cooperate with government OHS officials regarding workplace standards and codes of practice, benchmarks,
The annual security work plan aim to strengthen safety management, complete safety management objectives, strengthen safety awareness,effective control the various types of injuries and other major accidents, prevent or reduce the general safety incidents。
The security focus: makes “control people's unsafe behavior” as the core of the work, provide "safe working environment” for employees
1. Safety goals:
1) Serious injuries, serious security incidents (include a variety of quality accident and other major security incidents) is zero.
2) The officer sustained minor injuries, and other general safety accident rate does not exceed 50%.
2. Persist hold a safety committee meeting at least every quarter to analyze production safety and timely give information with about security problems in the production of hidden dangers; increased safety input and ensure the safety of capital investment throughout the year
Make sure all the policy can come true ; increase the rate of hidden rectification and ensure all the policy of control security risks can come true; develop and implement safe production responsibility system and make sure all the policy can come true.
3. Emergency plans and fire drills:
The company's total exercise: conduct an emergency evacuation drills and fire drills every first half year, in the second half year to conduct a fire drill.
Departmental exercises: exercises in various departments of not less than two times and record the process.
4. fire general project check:
1) Security is responsible for weekly fire inspection and records;
2) Generator checks , fire water system to detect and record information in time;
3) Inspection of fire-fighting facilities every quarter and rain system checks and records information in time and timely replacement fire-fighting facilities which are have been damaged;

5. Safety training:
1) Make sure every new employees are educated knowledge about safety operation and all of them have pass the examination for posts.
2) Conduct safety knowledge leaning meeting for older workers re-education monthly and make sure all the employees can come to the meeting, make sure the safety training hold at least two times every year;
3) Make sure all the external construction are educated the safety knowledge; Construction team must have construction certificate, submit techniques (security scheme), sophisticated construction safety organization and make the necessary security training prior to construction;
4) Built safety training system,it include laws and regulations, the group system, accident case, training, testing, training, operations, monitoring, tracking check and so on ;
6. The principle about implementation of the plan:
Make sure all the employees can respect safety system and OHS policies. Make sure managers are responsible for employees and employees are responsible for themselves.

Case
1. Introduction
XIANGNING, Shanxi - The number of people trapped underground after a coal mine flooding in north China's Shanxi Province was revised -- for a second time -- to 153 from 123, rescuers said late Sunday night.
Investigations showed that 261 workers were in the pit of Wangjialing Coal Mine under construction when underground water gushed in at about 1:40 pm. Of them, 108 were lifted safely to the ground while 153 others trapped in the shaft, according to the rescue headquarters.
"Most of the trapped are migrant workers from Shanxi, Hebei, Hunan and Guizhou provinces," a rescuer said.
The exact number of people trapped is still being checked.
Soon after the accident happened, the Shanxi Provincial Work Safety Administration and the Shanxi Provincial Emergency Affairs Office said 152 people were trapped underground, but officials later changed the figure to 123.
Rescuers are struggling to save the trapped people, and local authorities are investigating the cause of the accident.
President Hu Jintao and Premier Wen Jiabao ordered local authorities to spare no effort to save the trapped while guarding against secondary accidents.
Vice Premier Zhang Dejiang rushed to the site late Sunday to oversee the search and rescue operation.
The mine, sitting astride Xiangning County of Linfen City and Hejin City of Yuncheng City, covers about 180 square kilometers.
The mining zone boasts more than 2.3 billion tonnes of coal reserves, including nearly 1.04 billion tonnes of proved reserves, according to the company's official website.
It is now under infrastructure construction and is expected to produce 6 million tonnes of coal annually once put into operation.
The mine, affiliated to the state-owned Huajin Coking Coal Co. Ltd., is a major project approved by the provincial government.

2. Reason, result analysis & Hazard audit
Reasons: (1) Miners dug abandoned coal mines so deep that connected groundwater casing permeability. 2) When the mine started leaking water, high level didn’t pay attention to the report from miners.
Results: (1) The permeability has led to 153 people trapped under the coal mine (2) If the rescue isn’t effective, it may increase the risk of people death and injury (3) The permeability can cause a series of other dangers such as collapse (4) Beside physiological harms above, it also has a serious psychological effect on these miners who were trapped (5) The accident will have a bad influence on national image and increases difficulty of OHS management
