...Initiating Smart Goals Jennifer Clay Chamberlain School of Nursing Mary Thomas NR 447 Collaborative Health Care Introduction Smart goal #1; nursing will assess for appropriate use of indwelling catheters every shift in order to prevent CAUTI, nursing will obtain an order for discontinuation if no longer needed, this will be audited weekly in order to reach the goal of zero UTI’s on a monthly basis. This smart goal was developed to insure nursing is doing everything possible to prevent CAUTI. The goal steers toward patient safety and because Medicare and Medicaid do not reimburse for UTI’s acquired while hospitalized any longer. Smart goal #2; there will be 100% compliance in doing CVAD assessments, dressing changes, and the use of 70% isopropyl alcohol caps (CURO’s) in order to decrease the rate of CLABSI’s in the Medical-Surgical unit. This goal is to insure nursing is doing proper assessments, dressing changes, proper hand hygiene, using CURO’s caps and handling techniques in order to prevent contaminating the line. Goal 1: Preventing CAUTI Prevention of Hospital-acquired UTI’s should be a goal for every acute care setting to prevent the high cost of treatment, each episode of symptomatic CAUTI cost at least $600. It is not surprisingly the most common of health care associated infections accounting for about 40% of HAI’s. (Saint Et.al) This goal is to develop a way for nursing to assess the use of indwelling catheters and discontinue them if not needed to...

NURU THE PROPOSAL There were 15 million discharge summaries produced for admissions into hospital last year. A staggering 80% of these were found to be inaccurate or incomplete and another 70% of these were reported as being severely delayed on a regular basis. This compromise to clinical care and patient safety is simply unacceptable. Our empirical market research has found that the majority of junior doctors, the principal users of discharge forms, were unhappy with the current systems in place. It has also been reported that on average junior doctors spend more time carrying out admin duties than in formal training and teaching sessions. There are electronic discharge systems present however, these have been described as insufficient as they lack comprehensive coding and in some circumstances...

...we can indirectly improve our circumstance. In my perspective, this is a tool that falls under “control” (one of the three c’s). Control is believing that learning more skills can help you better the situation. SMART Goals SMART is an acronym meant to assist you to craft effective goals that will actually be accomplished. The acronym stands for: Specific, Measurable, Attainable, Relevant and Time Bound (the first letter, which is capitalized for convenience, is what makes up the SMART acronym). Specific is meant to help form a clear expectation, because if you are super vague you can easily allow yourself to get off easily instead of putting in all the work you had intended. Being able to measure the results allows a way to track your progress throughout. It also gives you a way to know when you have accomplished your task. Attainable is important because if you make a goal nearly impossible, then your motivation to accomplish said goal is likely to be non-existent. I know from experience that if I feel overwhelmed with a task, then I end up sitting there not making any progress at all. It would be a waste of time to create a SMART goal that you didn’t care about and that was something you never intended on doing. In summary, relevant is just saying to make sure the goal is going to be something that will actually help you in your life. ...
