Medical futility is a judgment that further medical treatment of a patient would have no useful result and that medical treatment whose success is possible, although reasoning and experience suggest that it is highly improbable. Deciding to withdraw care on a patient is difficult for all involved, however in some cases it is a necessary, humane, decision. Two very different examples are the case study in our reading, Mr. Smith, and of Terri Schiavo. Mr. Smith was an elderly man who had lived with Multiple Sclerosis for over 20 years. He was admitted to the hospital with urosepsis and his sons wanted everything done for their father. Mr. Smith did not have a living will. While initially Mr. Smith’s condition was treatable, he soon deteriorated to the point that he was in critical condition, his body shutting down, needing the aide of a ventilator, dialysis, and multiple medications to keep him alive. His grown sons agreed to change his code status but still wanted to continue treatment. In this instance, I believe medical care should have been withdrawn. Initially, I agree with the treatment plan, including his surgery, but when Mr. Smith made it out of surgery and his condition deteriorated was when his condition was a case of medical futility. While it is difficult to make a decision regarding the treatment of a parent, to me, it is extremely selfish to continue a life by artificial means. Today’s advances in healthcare make it possible to keep patients alive longer through various medications and treatments. Mr. Smith had lived with a crippling disease for over 20 years. Per the information given in the case study, it seems to me the sons were more interested in continuing to receive the special compensation their father received than they were about what was in the best interest of their dad. Terri Schiavo is a very different scenario. After collapsing at her home at the age of 26, Terri was deprived oxygen for several minutes causing brain damage. Terri could not take nutrition orally and had a feeding tube in place. Terri certainly wasn’t the same after her collapse. However, there are documented reports of her reacting to family members. Her husband wanted to withdraw her feeding tube so she could die “peacefully” but her parents wanted to keep providing medical treatment and care to their daughter. While Terri was considered to have brain damage and was in a wheelchair, she was able to breathe on her own and maintain the functions needed for survival (ie blood pressure, heartbeat, kidney function, etc.). To her parents, she was their daughter and deserved the necessary physical, occupational and speech therapy to help her reach maximum recovery. Her husband thought otherwise. He considered her a vegetable. He wanted the feeding tube removed so she could die. The saddest part about Terri Schiavo’s case is her parents were willing to take care of her but because her husband was next of kin, and Terri didn’t have a living will, all decisions were made by her husband. His decision was to remove the tube and sadly, Terri was starved to death. She died two weeks after her feeding tube was removed. Should she have died? Absolutely not, especially with her parents and siblings more than willing to assume all responsibility of her care. Withdrawing care of a patient with zero chance of survival is completely different than starving a person to death. Mr. Smith was 84, had been bed bound for 20 years, and couldn’t maintain any bodily functions needed for survival. Terri Schiavo’s only artificial means of survival was a feeding tube. One way all of these decisions could have been in the patient’s hands is if they each had a living will. In my opinion, when a person reaches 18, they should immediately have to fill out a living will in order to graduate high school; register for college; renew a driver’s license; and/or registering to vote. This will prevent unethical decisions regarding treatment and ensure wishes are followed. No one should have to suffer.
