As an Advanced Emergency Medical Technician the response and scene size up starts as any other calls (BSI, Scene safety, Number of Patients, nature of illness, and additional resources). Body substance isolation is always important. One thing to keep in mind the substance that caused the anaphylactic reaction could cause potential life threats to the emergency care provider (EX: medication that provider is also allergic too that needs to be removed). Another thing to keep in mind is the cause of the reaction could potentially make the scene unsafe for the emergency responders (Active beehive). Additional resources may need to be utilized for the mediation of the problem.

It is very common that patient that have been diagnosed with severe allergies to carry a self-administration epinephrine device. If the patient doesn’t have one, Advanced emergency medical technicians are licensed though the state of Alabama to administer Epinephrine 1:1000. Epinephrine slows the reaction by constricting the blood vessels with rapidly increasing the blood pressure, also increases the strength of the cardiac contractions in the body and dilates the bronchioles. Side effects of epinephrine include an increase of blood pressure and pulse rate, anxiety, nausea and vomiting, tremors, chest pain, and cardiac arrhythmias. Do not use the drug if it is discolored, out of date, or if it’s not the patients prescribed injector in the case of doctor prescribed. In the state of Alabama the AEMT is to give adult patients 0.3mg (0.3cc) via intramuscular injection offline. If the patient is older than 65 years of age, has a history of heart disease or uncontrolled hypertension the AEMT is to contact online medical control before administration. If the dose needs to be repeated, online medical control also needs to be contacted. There may also come a time in which pediatric patients will need epinephrine. In that case the state of Alabama protocol states that the pediatric epinephrine dose is 0.01mg/kg (0.01cc/kg) MAX of 0.3mg (0.3cc) intramuscular …show more content…

Before giving Diphenhydramine confirm with the patient that he or she doesn’t have known hypersensitivity to the medication. The medication usually causes sedation, however it may cause excitation in children. It has been documented that there has been sedation effects with alcohol or other central nervous system depressants. In the state of Alabama the AEMT is to give adult patients Diphenhydramine 50mg intramuscular injection or intravenous injection. There may also come a time in which pediatric patients will need Diphenhydramine. In that case the state of Alabama protocol states that the pediatric Diphenhydramine dose is 1mg/kg intramuscular injection or intravenous injection (Max 50mg).
