Assignment C (Observation of OT Process)
The client’s primary diagnoses are carpal tunnel syndrome, trigger finger in the right middle finger. Both primary diagnoses are located in the right wrist and hand. The client’s secondary diagnoses are diabetes type I and II, hearing impairment, and high blood pressure. This client is very compliant and follows directions at all time. Although he is in pain, he constantly tries to perform the activities being assigned by the therapist. He attended therapy three times a week and always on time. His goal is to become independent again to return to his daily occupations. The therapists known him for a while because he recently attended therapy at physiotherapy associates. During the first session the client worked on improving his wrist flexion and extension to increase his level of independence to perform his IADL and ADL tasks such as dressing, bathing, personal hygiene, and grooming. The performance skill addressed by the first session was fine motor skills. The client crumpled up ten sheets of newspapers, the therapist grade the activity by having the client crumple the paper with the affected hand. This activity will increase his ability to grasp objects in an appropriate manner so they do not slip. The client practiced picking marbles with a yellow plastic clothespins by putting each marble one by one in a jar which is helpful for increasing his ability in using his dexterous finger movements and three-jaw chuck. The activities involved repetition; therefore, the client needs to repeat each movements learned at home and not just during therapy session to increase his level of independence and to provide structure for his daily life. The client has difficulties handling, carrying, and moving objects from one place to the other. He also has difficulties performing his own self-care and other non-specified physical or occupational functional limitations. The client uses the left hand to compensate when lifting objects that are under 5 lbs to avoid further damage to the right hand. He is capable of feeding himself independently. V. Pt. will incorporate R LF into ADL task 75% of the time or greater with minimum symptoms.
The Goniometer is an instrument used to measure the amount of motion in a specific joint. During the initial evaluation the therapist took the measurements of the client’s right hand with a Goniometer to determine his available motion. On the first visit the therapist put an ice pack on the client’s hand to decrease the swollen in his right hand. As mentioned earlier, he received a trigger finger home exercise program which he practiced with the therapist to assure he understood and know how to perform it. He did the Barsteady to increase flexion and extension of the wrist.
The therapist interacts pretty well with the client. She spoke to the client with a warm voice. She responded to questions with respect and a professional tone. She demonstrated empathy, which allows a more open communication between the client and the therapist during the treatment session.
