Not only is it important to keep notes on each client but also for future reference on a client, a case study will come in handy. This paper will illustrate an example of a case study and examine the following, family, social, and intimate relationships of the person in the case study, identify any role changes that may have occurred, and the immediate and future effect of healthy and unhealthy habits practiced by the person. My case study is on Jackson. A 25 year old male with a substance abuse problem. Upon graduating high school, Jackson attended college the following fall to obtain a degree in Chemistry. During his freshman year of college, Jackson suffered a head injury after a car crash and resulted in the loss of cognitive and analytical functioning skills. Thus began drinking alcohol and abuse of painkillers. Jackson has left college and now lives with his mother while he attends a substance abuse program in Chicago, Illinois. They then questioned the boy about his injury and his response matched his fathers. However the boys injuries were clean and the story seemed false. When asked if he was scared of getting hurt at home his answer was no. Before leaving the hospital Tavions father requested that the medical records are released to no one, unless he was contacted. The following weekend Tavions mother, who is divorced, requested a copy of the hospital records because she suspected abuse. I do believe that Tavions mother has a right to the hospital records of her son, especially if she believes her son is being abused by his father. This does give the hospital the right to investigate further. Tavions mother knows the father of her son more than the hospital staff and if she has concerns it should be looked into. She is divorced, but is in a relationship with a man that is the same age as she is. Emergency room staff suspects that Tavion’s injuries did not happen as his father reported because the abrasions were too clean. Tavion’s abrasions would not have been so clean if brought in immediately after his fall from the jungle gym. While Tavion was undergoing his x-ray, the staff did not allow Tavion’s father in the room, and they were able to question Tavion about his injuries. Tavion verified his father’s story and consistently denied and abuse or fear of his father. While in Zambia, Dr. Marshall decides to go sightseeing and while out he collapses due to dehydration and suffers a significant laceration on his arm.
