Quality, safety, and efficiency need to be maintained during implementation. The case study reviewed: Impact of electronic health record implementation on patient flow metrics in a pediatric emergency department (Kennebeck, Timm, Farrell, Sooner, 2012) attempted to quantify the effects of offloading low acuity patients and its impact on length of stay. Length of stay and volume of patients in an emergency department are correlated. The study was conducted in a pediatric emergency department (ER) in Cincinnati, Ohio. Prior to implementation, a plan was utilized to off load patients to a different area in the ER due to a surge from the H1N1 flu virus. This was successful in decreasing patient numbers and length of stay. The organization decided to use the same template for the EHR implementation. Despite its prior success, offloading patients did not improve efficiency or length of stay (LOS) during EHR implementation. The primary issue identified was the failure of offloading patients. The purpose of this paper is to identify ethical issues within a case study. This case study involves an eight month old infant who presents to the emergency department with injuries. After arriving at the emergency department it is noted that the infant has bilateral fractured femurs. The ethical issue identified within this case study is the possibility of child abuse by either the mother or a worker at the daycare facility. The nurse makes notes of her concerns within the patients chart as well as verbalizes concern to the emergency room physician regarding the injuries to the infant. It appears to be a lack of follow-up with these concerns so this presents an ethical issue. There are eight ethical principles that a nurse should learn to apply to each situation that he or she feels there is an ethical decision that needs to be made. The eight principles to apply are autonomy, beneficence, nonmaleficence, veracity, fidelity, justice, paternalism, and respect for others (Guido, 2010). This paper undertakes a critical evaluation of the different methods used to assess the quality of provision in higher education departments in the UK. Methodology Drawing on relevant literature, the authors develop a quality audit tool that incorporates all key components of effective quality management programmes and apply it to a single UK case study department. Findings The findings suggest that the potential for quality enhancement is determined by the manner in which the evaluation is conducted and subsequent change implemented. Perhaps unsurprisingly there is currently an emphasis on internally derived quantitative data and there is potential to enhance the management of the quality of HE programmes. Research Limitations/Implications This article has concentrated on the development of the quality audit tool and tested this within one UK department. Work is underway to now test the tool on an international basis. Day, the data-processing vice president, examined the turnover, absenteeism, and productivity figures of all of the bank’s work groups. The results Day obtained offered no real surprises except in the case of the check-sorting and data-processing departments. is quiet and comfortable. Both groups are known to be highly cohesive, and the workers in each department function well with one another. This observation was reinforced by the study’s finding of the low levels of turnover and absenteeism. THE INTERVIEW DATA In an effort to understand this phenomenon, Day decided to interview the members of both departments in order to gain some insight into the dynamics of each group’s behavior. Day discovered that the checksorting department displayed a great deal of loyalty to the company. Most of the group members are unskilled or semiskilled workers; although they have no organized union, they all felt that the company had made special efforts to keep their wages and benefits in line with unionized operations. They knew that their work required team effort and were committed to high performance. A quite different situation existed in the dataprocessing department. The study population comprised 4,570 students, 65 head teachers, 65 deputy head teachers, and 65 heads of Guidance and Counseling Department from all the 65 secondary schools in the District. Out of this, a sample of 22 head teachers, 22 deputy head teachers, 22 heads of Guidance and Counseling and 916 students from 22 secondary schools was selected through the simple random sampling technique. Two instruments were used to collect data for the study. These were interview schedule and a questionnaire. The data collected through questionnaire were analyzed using descriptive statistics in form of frequencies and percentages. While data collected using interview schedule were audio taped and transcribed into themes, categories and sub-categories as they emerged from the data. They were analyzed using summary Tables for the purpose of data presentation and interpretation. The findings show that guidance and counseling was minimally used to promote student discipline in secondary schools in Kisumu District.
