Slide 2: After taking the initial belbin team roles test and extensive group discussion we came up with the following roles: I was chosen as the chairman because of having comparatively better knowledge of shares and stock exchange. As - was quite organized, she was better at maintaining agendas and minutes, hence being perfect for the role of secretary. - had a regular access to research material making him a good researcher. After a number of weeks, as a group we decided to switch - as the treasurer and - as a researcher, as these roles best fit their traits and abilities. Like every other share investment club, success is defined by the amount of profit made. Hence, we did end up making a considerable amount of profit. And for that reason initially we invested in 3 safe while only 1 risky company for giving a stable point for our investment club. The Therac-25 was involved in six massive overdoses that cost three people’s lives, while the machine gave the operator error messages or a message that states that no dose was given (Baase, 2008, p. 425). After the first accident, some users became skeptical of the safety of the device. Yet, the manufacturer turned a blind eye on the matter. They responded to the incident that they have fixed the problem. In the following months, five more accidents occurred. In each incident, the manufacturer, the computer programmer, and the clinic/hospital using the Therac-25 have some responsibilities and flaws that need to be addressed. The Problems Manufacturer What would be the first thing that a manufacturer needs to do when they have an incident with one of their products? It should be the recall of that product, in this case the Therac-25. The Therac-25 can help a person ailing with cancer with its beam but the beam itself would be too harmful to the patient. The fly-fix-fly approach was a way of how engineers and manufacturers used to detect flaws and issues with an aircraft’s design and construction by actually flying the aircraft. Every flight led to a new discovery or a new fixture to a fitting which was something the engineers would work on before it was flown again. This method is commonly known as the trial and error method. After many years of development and with the advancement of technology, programs like space travel and/or nuclear weapons for instance had to be first-time safe which made the fly-fix-fly approach unacceptable. As we walked through the plant, he pointed several of them out and what exactly each machine does. Some of the machines were responsible for machining large pumps, whereas others were specifically designed for smaller operations. The variety of the machines as well as the pumps and valves was amazing. Our tour guide showed us some that were small enough to hold in your hand and go on regular water or air lines, and he also showed us some of the bigger pumps which had an eighteen inch diameter. One thing I found rather interesting was the price difference from what it costs to build one of the valves or pumps and the price that the customer is willing to pay for the product along with proper documentation.
