Associations Among Health Literacy and Outcomes in Pregnancy: A Systematic Literature Review

Pregnancy and childbirth is often a woman’s first adult encounter with the health care system (Ferguson, 2008). Pregnant women need to have the necessary knowledge, skills, and attitude to best interact with the complex health care system. Outcomes such as low birth weight, premature delivery and breastfeeding rates have significant influence on a child’s health as well as increased cost to the system. The medical cost of a preterm birth is approximately three times more than a term birth. In 2005, the estimated cost associated with preterm birth was $26.2 billion when accounting for medical, educational and lost productivity costs (Institute of Medicine, …show more content…
However, there is limited research examining the relationship between health literacy and outcomes related to pregnancy. Health literacy is important because it is potentially a modifiable variable within the health care system. Changing someone’s economic status or educational level is out of healthcare providers’ hands. However, adapting communication, care and patient education in regards to a patient’s health literacy level is feasible. The goal of this paper is to analyze the current body of literature on health literacy’s relationship with prenatal outcomes and identify areas for future research. This paper will also synthesize from the literature which interventions may be most appropriate for narrowing the gap between patients with low and high health literacy. Initiating and sustaining breastfeeding was significantly increased in patients with higher health literacy levels (Kaufman, Skipper, Small, Perry, & McGrew, 2001). Endres et al. (2004) found that patients with pregestational diabetes and a low health literacy level were less likely to discuss getting pregnant with their providers. Women with higher levels of health literacy and diabetes in pregnancy were more likely to record their glucose readings throughout their pregnancy (McLaughlin, 2009). Bennett et al. (2006) found that there was inadequate utilization of prenatal care in 50% of this sample, but there was no difference in regards to literacy
