This paper addresses the population of adults with diabetes in the hospital setting and focuses on the related topics of health literacy and health education. The first section of this paper will discuss the relationship between these three topics. Next, this paper will outline goals of care, how nurses can work to ensure continuity of this care, and the transition of patients from the hospital setting to outpatient facilities.
The World Health Organization defines health literacy as “the cognitive and social skills which determine the motivation and ability of individuals to gain access to, understand and use information in ways which promote and maintain good health” (WHO, 2015), making health literacy an important factor in the manage …show more content…
This can be done through the use of assessment tools, one of which being the Rapid Estimate of Adult Literacy in Medicine—Short Form (REALM-SF). The REALM-SF uses a 7 word recognition test to give clinicians a quick, accurate assessment of patient’s health literacy (AHRQ, 2014). This brief assessment can help practioners tailor their teaching to the appropriate literacy level-making the chances of successful learning and adherence to treatment regimens more likely. In a study published by the Journal of Health Communication, researchers sought to link health literacy, self-efficacy and numeracy with glycemic control. They found that there was a significant association, reporting that health literacy and patient awareness were negatively correlated to diabetic control (Tang, et al., …show more content…
Using the information gathered from the previous goals of care, diabetic health teaching can be adapted to the patient’s literacy level and current disease management abilities. There has been a longstanding theory that patients in the hospital are “too sick” to be taught about their disease. In discussion with Jo Carol Driver about inpatient diabetes education, this was her sentiment exactly. She stated “we know that inpatient diabetes education is ineffective. There is a bunch of research on that topic” (J. Driver, personal communication, March 3, 2015). And while yes, there is a lot of research on this topic, is it naïve to believe that all patients will follow-up with outpatient education programs. The patients who are being admitted time and again for diabetic complications are the ones who are typically (for a variety of reasons) not going to make it to the outpatient clinic. So then isn’t it the responsibility of hospital healthcare personnel, as likely the only point of health educator contact, to teach these patients how to manage their disease? And is all that research about not providing inpatient teaching really all that accurate? According to Sara Healy and colleagues, who explored the relationship between inpatient diabetes education (IDE) and hospital readmissions in patients with poorly controlled diabetes, this isn’t as true as many believe.
