...The role of an occupational therapist is of vital importance in order to ensure safety and promote independence in all aspects of a patient’s life. During this placement I had the opportunity to work with the occupational therapist and also accompany her during a home visit. A few of the health conditions that I came across that benefit from occupational therapy include limitations following a stroke or heart attack, arthritis, multiple sclerosis, or other serious chronic conditions, spinal cord injuries, or amputations, broken bones or other injuries from falls. Occupational therapists provide a service that is customized to improve a person's ability to perform daily activities. Occupational therapists use a variety of interventions including biofeedback, relaxation, goal setting, problem solving, planning in order to rehabilitate patients. During the home visit we visited an 87year-old gentleman with a history of falls, cardiac failure, atrial fibrillation and dementia. The patient felt dizzy when he stood up and at one point burned himself while standing next to the radiator and did not notice the injury, he also struggled to carry out activities of daily living like getting in and out of the bath, in and out of bed and going up the stairs. The occupational therapist played a fundamental role to ensure safety of this patient. The patient was offered a hand rail in the bath to help when standing up, rails along the stairs so he could hold on to when going up and down the stairs...

...personal information remains daunting. Similarly, employers attempting to assist the process are often awed by the extent of collaborations involved in integrating employees with mental health issues back to work as well as concern about compliance with human rights legislation. Needed accommodations in terms of approach to the work itself are often simple; however substantiating the need for adjustments is more complex. This case study introduces a model to support the development of shared goals and shared understandings for return to work (RTW) among workers with mental health concerns, employers, co-workers and therapists. The model of occupational competence is used as a basis to guide dialogue, identify challenges and generate solutions that take into consideration a worker’s preferences, sensitivities, culture and capacities in relationship to the occupational demands in a given workplace environment. A case study is used to demonstrate the potential utility of the model in assisting stakeholders to strengthen collaborations and partnering to achieve a shared understanding of worker and workplace needs. 1. Introduction Disclosure of mental health issues in the workplace is influenced by multiple factors including intrinsic and extrinsic issues. While there is legislation that protects a worker’s right not to disclose health information in the workplace, some workers may want to share aspects of their challenges....

...Risk Management Health care institutions are the centers for different kind of healthcare services, so when patients and their families come for check-ups, surgeries, acute hospitalizations, tests, minor procedures, etc. they expect quality care and positive outcomes. Issues come up when patients’ expected outcomes are not realized due to one reason or another. Factors that may prevent positive outcomes in patient care may be quality management issues or risk management issues. Before a full introduction and analysis of risk management in my place, one has to understand quality management and risk management. In terms of quality management, they are issues that arise in the work place as a result of poor management and need to be improved on. They are more prevention oriented (Sullivan and Decker, 2009). In the case of risk management, it is about issues that are problematic throughout a health organization and may lead to poor patient outcomes. As described by Sullivan and Decker (2009), “risk management’s purpose is to identify, analyze, and evaluate risks and then to develop a plan for reducing the frequency and severity of accidents and injuries” (p. 84). Risk management is not about monthly, quarterly or yearly assessment and improvement of programs, but a daily assessment of issues that arise in the work place that may lead to bigger problems if not corrected. Current risk management issue at my work place. The main current risk management issue in my place of employment...

...Falling in adults 65 and older is a complex problem confronting public health, the health care system and families. Statistics alone do not begin to measure the pain, suffering and loss of independence that are experienced by older adults who fall, but a number of trends highlight the magnitude of the problem: o Falling accounts for 80-95% of hip fractures in older adults. o The rate of fatal falls increases dramatically with age. o Falls are the leading cause of injury deaths for older adults. o Among adults 75 and older, those who fall are four to five times more likely to be admitted to a long-term care facility for a year or longer. (Mission Hospital Data, Risk Management, 2012) For decades, hospitals and other health care organizations have integrated to understand the contributing causes of falls, to minimize their occurrence and resulting injuries or deaths. Today, organizations have begun reaching out to each other for collaboration on the best ways to prevent falls. Based on Centers for Disease and Prevention (2012) data, each year one in three older Americans (65 and older) falls and about 30% of those falls require medical treatment. Falls are not only the leading cause of fatal and nonfatal injuries but also the most common cause of hospital admission for trauma. More than $19 billion annually is spent on treating the elderly for the adverse effects of falls: $12 billion for hospitalization, $4 billion for emergency department visits, and $3 billion for outpatient...

...Managing Improvement Understand the Effectiveness of the Organisation and own Ability to Manage and Improve Quality to Meet Customer Requirements Critically assess the organisations effectiveness in managing quality to meet or exceed customer requirements There are several Welsh Government legislative guidelines that influence quality within healthcare at both strategic and ground level. Everyone who works in or for the NHS is there, first and foremost to serve the public. Therefore, everyone at every level has a part to play in driving up standards of safe, effective, patient-centred care. The consistent delivery of safe, high quality care relies on contributions from a wide range of organisations, individuals and stakeholders. The Welsh Government’s, Achieving Excellence: The Quality Delivery Plan for the NHS in Wales (2012/2015) sets out their ambitions for achieving excellence in Welsh Healthcare by 2016. Their vision is for a quality driven NHS, focused on providing high quality care and excellent patient experience. These standards “are key to underpinning the vision, values, governance and accountability framework for the new NHS Wales” and are seen as a key tool, alongside the guidance from the 1000 Lives Quality Improvement Campaign and other initiatives in helping to drive up clinical quality and patient experience. The aim is been to map the quality standards more closely with service specific and professional standards and quality requirements such as the...

...Discharge planning is a process that aims to improve the coordination of services after discharge from hospital by considering the patient’s needs in the community. It seeks to bridge the gap between hospital and the place to which the patient is discharged, reduce length of stay in hospital, and minimise unplanned readmission to hospital.1 Discharge planning is an established part of hospital care, but the process varies and is not entirely evidenced based. A Cochrane review analysed 11 randomised controlled trials looking at discharge planning in over 5000 patients and failed to show a reduction in mortality among elderly medical patients, lower readmission rates, or a shorter length of hospital stay.1 However, two trials in the review did report greater satisfaction of patients and carers when discharge planning was used.2 3 The Cochrane review concluded that discharge planning remains important as a small improvement, not detected by the studies performed so far, could still yield highly significant gains in health care with huge resource implications and better use of acute hospital beds.1 Unfortunately, none of the included trials assessed communication with primary care staff about patient transfer of care. This is an important aspect of discharge planning and another potentially important advantage for patients. On a patient’s initial contact with health services, discharge planning should be started.4 This is often difficult to achieve when acutely unwell patients...

...Running head: IMPROVING TEAM COLLABORATION April 9, 2015 Improving Team Collaboration As a home health care nurse multi-disciplinary collaboration regarding a specific patient can be very challenging. With the increasing number of complex patients, patients can have care provided from several different professionals with very different goals. These disciplines include: skilled nurse, physical therapy, occupational therapy, speech therapy, medical social worker, and home health aide. The lack of consistent communication between health care providers can lead to poor outcomes, such a falls, decreased patient satisfaction, and unplanned hospitalizations. For example if a patient has a medication change causing weakness or unsteady gate that is not communicated to the therapist, the therapist may send patient to hospital concluding it is a functional change instead of a medication side effect. This is just one of many things that can occur when poor communication occurs within a multi-disciplinary team. As previously mentioned home health care has seen a significant growth of increasingly complex patients requiring multiple disciplines in the home. Each member of the healthcare team has patient centered goals. It is necessary for each discipline to be aware of the goals that each discipline is working on to form a seamless plan of care. Due to the increased complexity of the plan of care it is necessary for the team to meet via conference call or in person to collaborate...

...SITUATIONAL ANALYSIS OF THE ROLE OF THE STAFF NURSE IN THE CONTEXT OF INTER-PROESSIONAL WORKING. The aim of this assignment is to discuss the role of the nurse in the context of the inter-professional team. Discharge planning will be discussed and analysed incorporating relevant knowledge such as teamwork and role enhancement. Relevant social, economic and health policies such as clinical governance will be discussed. Perspectives of each of the principle professional groups including myself as a third year student nurse, will be incorporated and collaborative team working addressed. The Department of health (2002) describes discharge planning as a pathway used to decide patient needs, for a smooth move from one level of care to another (Hunt 1996) and is the process through which patient’s needs are identified and plans are written, facilitating continuity of health care (Jackson 1994). According to the department of health (2000) discharge planning illustrates well, the role of the nurse within the inter-professional team the balance between goals set, care delivered and evaluation of care from a nurse and team perspective. Nurse involvement is considered integral and, aims to provide effective discharge plans (Department of Health 2000). The Royal College of Nursing (RCN 2002) supports the concept of nurses taking the lead and recommends the development of nurse led protocols in discharge planning. However the NMC (2004) highlights the importance of nurses being able...
