A patient record has two components: clinical data and administrative data. Clinical data is data that pertains to patient diagnosis, treatment, and patient progress and is documented by the attending or primary physician. It provides the hospital with preliminary information about the patient befor they are admitted. Administrative data is data that pertains to patient demographics, insurance, consents, and authorizations and is collected by the clerk. The demographic information is used to study statistic characteristics of the human population. Consent to treatment forms document the patient's permission to routine care before care is provided, while authorization forms document the patient's formal permission to release confidential health
