Personal Narrative: A Day At The Emergency Department

I look forward to the evening that I spend in the emergency department, when going into each room my goal is to make each patient and their family as comfortable as possible while hopefully bringing a smile to their face despite their circumstances. My long conversations with them reveal that each person is more than what meets the eye, each patient is abound with stories of pain and joy while sitting on a bed waiting for treatment.

Every shift in the ED I began with an open mind and the great potential to make a positive impact on someone during a time of hardship. Although each action itself seemed miniscule to me such as sitting with a 90 year old woman as she tells me stories of her great-grandchildren, bringing toys to a toddler to distract

Technology in Healthcare

...Healthcare Abstract This paper explores nursing informatics and technology in the emergency department of a local level 1 trauma center. The references cited provide discussion and statistics on several types of nursing informatics utilized today by this facility, including benefits and shortcomings. Personal experience from the author are also incorporated as he witnessed first hand the nursing informatics and technology “explosion” and how this facility incorporated this technology into their daily nursing care over the past 15 years. In 1995 I acquired a job as a nurse’s aid at the hospital I am currently employed. At the time, they were just introducing computers at the nurse’s stations. It was used primarily for their first generation of Computer-Assisted Physician Order Entry (CAPOE). With exception to limited order entry, all the documentation was still placed on paper charting. Difficulties I encountered first hand included tracking down the chart while other staff members had possession of it. It made charting in a timely fashion almost impossible, and I often found myself charting late entries. Today I am a Certified Emergency Nurse in the Emergency Department of LVHN. In the grand scheme of things, 15 years is relatively a short time, yet the advances in technology I have been witness to are sometimes overwhelming. “Informatics is a field that’s evolving every day, and there’s much to learn” (Abbott, 2002 p. 14). Today, the majority of charting is...

Do X-Rays Requested by Triage Nurses Decrease Waiting Times?

...Introduction: Since 1991 when the patient’s charter (Department of Health 1991) was introduced, Accident and Emergency (A&E) departments have been under pressure to see, treat and discharge patients within a certain time period. In 1991 75% of walking wounded who presented to the department had to be discharged within one hour. It was subsequently changed in 2004 when the government decided that 98% of all patients attending an A&E department should be in the department no longer than four hours (Department of Health 2004). And again in 2011 when the department of health introduced the clinical care indicators that relaxed the target to 95% seen, treated and discharged or admitted in 4 hours (Department of Health 2011). Triage nurses within the accident and emergency department are one of the first people to assess the patients who present with an injury or illness. The patient’s condition is assessed and their need for how quickly they require treatment is prioritised. Triage nurses are usually experienced A&E nurses who have many years A&E experience (Dolan et al 2007). Patients are then placed in queues to await assessment by a Doctor or Emergency Nurse Practitioner. Once seen they are then put in another queue to await x-ray if necessary before waiting again to see the doctor/ENP that will interpret their x-ray and decided on the best course of treatment (Ward 1999). Many patients have long waits within the department for x-rays and assessments, and this can increase...

Flextime

...Introduction to Flextime Flextime is defined by businessdictionary.com as the non-traditional work scheduling practice which allows full-time employees to choose their individual starting and ending times within certain limits (such as 'not earlier than 5 a.m.' and 'not later than 9 p.m.'). Flextime periods usually precede or follow a core time during which all employees must be present. According to history records, flextime was first suggested by Christel Kaemmerevback back in the year 1956 and was first implemented in West German businesses in 1967. Flextime was introduced as an intervention to reduce tardiness and absenteeism which resulted in 40% reduction in absenteeism, along with decreased overtime expense, decreased turnover rates, decreased tardiness, and increased morale. The practice of flextime spread quickly through Western Europe due to a high demand from workers and less governmental restrictions on work schedules. At the present time, due to the advancement of information technology and information system, this has further maximize the flexibility of work during the core hours to be stretched to working during non-core time like the weekends by enabling employees to work remotely from home instead of being physically present at the office. Flextime is no longer just considered as a benefit or privilege for the employees, but is able to provide competitive advantage for the organization over its rivals by being more flexible as a whole by providing quicker and...

Studies in Professional Life and Work

...STUDIES IN PROFESSIONAL LIFE AND WORK Mike Hayler University of Brighton, UK Autoethnography, Self-Narrative and Teacher Education examines the professional life and work of teacher educators. In adopting an autoethnographic and life-history approach, Mike Hayler develops a theoretically informed discussion of how the professional identity of teacher educators is both formed and represented by narratives of experience. The book draws upon analytic autoethnography and life-history methods to explore the ways in which teacher educators construct and develop their conceptions and practice by engaging with memory through narrative, in order to negotiate some of the ambivalences and uncertainties of their work. The author’s own story of learning, embedded within the text, was shared with other teacher-educators, who following interviews wrote self-narratives around themes which emerged from discussion. The focus for analysis develops from how professional identity and pedagogy are influenced by changing perceptions and self-narratives of life and work experiences, and how this may influence professional culture, content and practice in this area. Autoethnography, Self-Narrative and Teacher Education Autoethnography, Self-Narrative and Teacher Education STUDIES IN PROFESSIONAL LIFE AND WORK The book includes an evaluation of how using this approach has allowed the author to investigate both the subject and method of the research with implications for ...

Knott, A. & Kee, C. C. (2005). Nurses’ Beliefs About Family Presence During Resuscitation. Applied Nursing Research, 18(4), 192-198.

...family member in the PACU, only 47% of staff believe that families should have the option to visit in their own PACU. The perceived barriers reported by staff were staffing issues, the possibility of exposure to infection, privacy issues, staff anxiety, the possibility of visitors witnessing resuscitation, and lack of education of families. The survey results show that more existing education is needed. Consequently, the current policy is posted in all waiting areas for families, and a mandatory in-service was created and presented to staff on how to communicate effectively with family members. Keywords: family visitation, staff attitudes, perceived barriers, PACU. Ó 2009 by American Society of PeriAnesthesia Nurses ALTHOUGH MANY EMERGENCY DEPARTMENTS around the country are allowing family members at the bedside during resuscitations, there is resistance to family visitation in the postanesthesia care unit (PACU). Family visitation benefits both families and patients,1 and at a time when competition for surgical patients exists, hospitals must continue to improve patient and family satisfaction. The PACU where this project was conducted had no formal visitation policy. Reasons for having no formal policy, according to staff, were lack of privacy, lack of space, fear of families witnessing resuscitative efforts, and staff anxiety. As empirical evidence...
