The Nurse as a Change Agent in the Implementation of Electronic Health Record (EHR).
The use of health information technology in nursing continues to rise substantially with the sole aim of improving patient care, and outcome while curbing healthcare cost. Bates (2010), noted that the Federal government investment to promote EHR is likely to approach $50 billion. The use of EHR in clinical settings has numerous advantages. Although EHR depicts clinical charting and medical records, its inference is broad, ranging from communication between nurses, quality care, and the research surrounding EHRs that will inform nursing practice in the future (McGonigle & Mastrian, 2015 p.249). As first line caregivers, nurses have always collected data, interpret and communicate with the interdisciplinary team. The communication process in the initial phase of implementing HIT can be very complicated. According to Barrett & Stephens, (2017) one reason for resistance in EHR, implementation is the fact that it is a “radical” change as compared to a routine, change because it “stipulates new processes and components that represent a lucid departure from the status quo” (p.1006). Despite this radical change, the advantages of EHRs cannot be ignored. Heart, Ben-Assuli, & Shabtai, (2017) noted that EHR store patient information and are used by the physicians and the interdisciplinary team to improve the quality of care and contain costs. Cutting healthcare cost is important particularly in an era when affordability of health care is the major debate topic. EHR presents a story of the patients stay in the hospital, and it is readily available to every member of the interdisciplinary team.
Another advantage of HIT that can make nurses buy into the system is the use of personal Health records (PHR) with this system; patients will have access to their medical records in the comfort of their homes. Rodger, (2003) states that the adopter must understand how the innovation aligns with current practices. Technological advancement is a driving force in the rapid change in healthcare. There is no gainsaying that the rapid change in techniques and technology is impacting healthcare as seen in a breakthrough in data collection, research, and new treatments. The innovations are crucial because it helps health care professionals track valuable data for disease control, billing and reduce the use of unnecessary paperwork. According to Risling (2017), “improved access and quality of patient data are supporting additional technological advancements such as electronically integrated care planning and the increased availability of clinical decision aids for both patients and providers” (p.91).
Innovations are in line with current practice because there is the need for improved software to keep up with the new techniques and technology in nursing. Risling (2017) further notes that the new wind of change in healthcare requires a “shift in how nursing prepares future generations of practitioners and these are only a few areas where technological change is imminent”

The case study reviewed: Impact of electronic health record implementation on patient flow metrics in a pediatric emergency department (Kennebeck, Timm, Farrell, Sooner, 2012) attempted to quantify the effects of offloading low acuity patients and its impact on length of stay. Length of stay and volume of patients in an emergency department are correlated. The study was conducted in a pediatric emergency department (ER) in Cincinnati, Ohio. Prior to implementation, a plan was utilized to off load patients to a different area in the ER due to a surge from the H1N1 flu virus. This was successful in decreasing patient numbers and length of stay. The organization decided to use the same template for the EHR implementation. Despite its prior success, offloading patients did not improve efficiency or length of stay (LOS) during EHR implementation. The primary issue identified was the failure of offloading patients.
